// RGB of the Monster you want to attack
if Color.At coordinate is (RGB)("82", "127", "113", "1020", "62")
begin loop()
//Insert Attack Keys Here
Keyboard.Hold keys("x")
Keyboard.Release keys("x")
// Same co ordinates of the previous check
if Color.At coordinate is not (RGB)("133", "26", "19", "849", "59")
begin
Macro.Restart("no")
end
end

you could use the same script structure as DerMatze's but simply use pixel pattern instead of RGB at coordinates, like this:
Code:
 begin loop()
     Keyboard.Hold keys("{<tab>}")
     Keyboard.Release keys("{<tab>}")
     if  Color.Pixel pattern can be located on screen("", "")
          begin loop()
               Keyboard.Hold keys("")
               Keyboard.Release keys("")
               if  Color.Pixel pattern can not be located on screen("", "")
                    begin
                         Macro.Restart("")
                    end
          end
 end

However, it's gonna be tricky :) you'll have to set the pattern on the very last portion of the monster's HP bar (something like from 0% HP to 5 pixels up the HP bar)

You should look through BEM's documentation pages, located here: viewforum.php?f=135

What you have is close:
Code:
// color found in my screen click it ///
begin
if color.can be located on screen //look through the documentation pages I linked above
begin
     Mouse.Click at color (RGB)("246", "106", "101", "0", "left")
* you will probably need to add some attack
* and you'll probably want to add a macro.restart here too (depending on what else you're doing)
end //need an "end" for each begin
// search for monster //
else //there is no else, so you can delete this part
    Keyboard.Press key("{tab}")
end
BEM is read linearly, so if the color cannot be located on screen the macro won't enter the second "begin/end" part and will skip to "tab"...that's why you don't need an "else" in there.

See if that helps

It's Tree of Savior. He's got another post so I'll merge them since it seems to be the exact same question.
